| Offer of Additional Personal Accident Insurance Coverage for Members of Local 258 IBEW DA Townley & Associates is offering an self-paid additional “Personal Accident Insurance Plan” for members of IBEW 258. Details of the Plan: The “Personal Accident Insurance Plan” offers financial protection if you or a member of your family are seriously injured or die as a result of an accident. This plan covers accidents on or off the job, as well as travel anywhere in the world. You can choose a benefit of $100,000.00, $200,000.00 or $300,000.00. Coverage is available for you alone or you may also cover your spouse and children. (“Children” means dependent upon you for support and under age 23; or under age 26 if attending an institution of higher learning; or any age of mental or physical infirmity.) Enrollment is automatic you and your family can be covered regardless of present health or family history. The plan is paid by you. DA Townley & Associates will arrange for the cost to be set up as a monthly deduction from your chequing account. There is no obligation you may cancel your participation at any time. Coverage Description You are insured 24 hours a day, worldwide. This coverage provides lump sum, tax-free benefits in event of an accident causing death, loss of (or loss of use of) limbs, loss of sight, speech or hearing. The amount of your coverage is doubled for paraplegia, quadriplegia and hemiplegia. Coverage Options and Costs Coverage is available in units of $100,000 (minimum $100,000, maximum $300,000). The monthly SINGLE rate (member only) is shown below per $100,000 of coverage, and the monthly FAMILY rate (member and dependents) is shown below per $100,000 of coverage. Note: If you have selected the Family Plan and you do not have children, your spouse will be covered for an amount equal to 60% of the Principal Sum you have selected for yourself.
